Error Risk Reduction: Concept and Case Study

With the first case study, during the filling of a batch of an aseptic liquid product a change of the receptacle used for holding rubber stoppers (a ‘hopper’) was carried out within the aseptic processing area.  After unloading the stoppers, three clamps, three seals and a stainless steel ‘T’ piece were found to have been left inside the hopper.  A deviation report was raised, and an investigation performed in order to examine the reason why the error happened and with a view to prevent the error from happening again.
